Expands Arizona's immunization reporting system from children only to include adults, requiring health care professionals to report adult patient immunization data to the Department of Health Services

Requires reporting of adult patient name, address, social security number (if not confidential), date of birth, vaccine type, and administration date

Maintains existing confidentiality protections, allowing release of identifying information only to the patient, their health care decision maker, health care providers, insurers, schools, and designated health information organizations

Preserves the opt-out provision allowing any patient (or parent/guardian for children) to request their immunization information be withheld from the system before vaccination

Retains penalties for non-compliance, including professional conduct violations for health care providers and class 3 misdemeanor charges for unauthorized disclosure of confidential information
