Applies to all group contracts and policies except those issued to small employers already subject to existing disclosure requirements.

Allows health plans and insurers to make contracts contingent on the group contractholder's or policyholder's signature of the separate disclosure.

Specifies no state reimbursement is required for local agencies or school districts because compliance costs result from changes to penalty provisions for existing crimes.
