Requires the Director of General Services to notify state agencies that demographic data collection forms should include voluntary self-identification questions relating to sexual orientation, domestic partnership status, and gender identity (including current gender identity and gender assigned at birth)

Applies to public-use forms that collect demographic data during the usual course of reviewing and revising those forms

Adds these demographic data requirements to the existing State Forms Management Program under Government Code Section 14771

Removes prior requirement for a study of forms to be conducted and reported to the Legislature by January 1, 2012
