Exempts dogs from mandatory rabies vaccination if a licensed veterinarian determines on an annual basis that vaccination would endanger the dog's life due to disease or other considerations.

Requires exemption requests to be submitted by the veterinarian on an approved form and signed by the dog owner acknowledging understanding of consequences and accepting all liability.

Mandates that exempt dogs be confined to owner's premises or, if off-premises, kept on a leash not exceeding 6 feet under direct adult control, and prohibits contact with unvaccinated animals.

Limits license periods for exempt dogs to one year maximum, whereas vaccinated dogs may receive licenses up to three years.

Declares that no state reimbursement is required for local agencies implementing this act because they may levy service charges or fees to cover costs and because the act changes the definition of a crime.
