Prohibits health care service plans and insurers from requiring dentists to accept plan-set payment amounts for dental services not covered under an enrollee's or insured's contract or policy, effective for contracts issued, amended, or renewed on or after January 1, 2011

Requires dentists to charge no more than their usual and customary rate for noncovered dental services, with the Department of Managed Health Care and Department of Insurance not required to enforce this provision

Mandates that evidence of coverage and disclosure forms for dental plans and policies issued, amended, or renewed on or after July 1, 2011, include a specified notice informing enrollees or insureds that participating providers may charge their usual and customary rate for noncovered services and explaining the obligation to provide treatment plans with estimated costs

Defines "covered services" or "covered dental services" to include dental care for which the plan or insurer is obligated to pay, or would be obligated to pay absent contractual limitations such as deductibles, copayments, coinsurance, waiting periods, or frequency limits
