Requires the California Health and Human Services Agency to direct appropriate departments to apply for federal community transformation grants under the Patient Protection and Affordable Care Act (PPACA) Sections 4002 and 4201

Grant applications must support preventive health activities including healthful school food options, physical activity infrastructure, nutrition and smoking cessation programs, mental health initiatives, and worksite wellness programs

Mandates the agency prioritize strategies to reduce racial and ethnic health disparities and develop evidence-based prevention programming to reduce chronic disease rates

Requires the agency to make information about federal PPACA grant opportunities available to local governments, public health departments, school districts, nonprofits, and Indian tribes through existing channels and department websites
