Requires claimants whose claims include past or future medical treatment to provide social security number, Medicare health insurance claim number (HICN), alternate names, date of birth, and gender within 30 days of public entity request to assist with Medicare Secondary Payer Mandatory Reporting

Mandates claimants complete a Medicare Beneficiary Inquiry Form upon request of the public entity or its representative, as specified by Centers for Medicare and Medicaid Services guidance

Adds new subsection (g) to Section 910 of the Government Code establishing these information disclosure requirements for government tort claims involving medical treatment

Applies these requirements to claims presented to public entities for money or damages that include medical treatment costs
