Establishes a pilot project in up to 5 consenting counties beginning January 1, 2011, allowing severely impaired IHSS recipients to choose between receiving services through a public authority or a voluntary nonprofit/proprietary agency

Requires participating counties to administer IHSS through a public authority and contract with a voluntary nonprofit or proprietary agency for recipient choice options

Mandates contracted agencies maintain 24/7 emergency services, replace providers within 2 hours when needed, employ preferred providers when possible, and pay wages/benefits equivalent to public authority collective bargaining agreements

Requires each county pilot project to run for 4 years with an independent evaluation by the end of year 3 assessing consumer satisfaction, cost-effectiveness, provider turnover, worker satisfaction, and fraud identification efforts

Specifies that state does not pay administrative costs for the pilot project, with costs instead included in county contracts and paid by the voluntary nonprofit or proprietary agency
