Authorizes the State Department of Health Care Services to provide specific information about eligible Medi-Cal enrollees with HIV/AIDS to contractors who deliver disease management services, provided signed informed consent forms are obtained from each person whose information is disclosed.

Limits disclosable information to contact information, names and contact information of health care providers, and number of visits to providers in the past 18 months, explicitly excluding treatment information, billing codes, and other confidential provider-patient information.

Requires contractors to use only department-approved outreach materials, implement confidentiality safeguards, prohibit sharing protected health information with subcontractors, and obtain enrollee consent before releasing confidential information.

Prohibits contractors from using outreach activities to market other services they provide, though enrollees may voluntarily use other contractor services.

Establishes informed consent process requiring written forms that explain consent is voluntary, not a condition of Medi-Cal eligibility, and allow enrollees to revoke consent at any time.
