Requires health care practitioners licensed under Division 2 of the Business and Professions Code to disclose their name, license type, and highest academic degree to patients either in writing at initial visit or in prominent office display.

Physicians and osteopathic physicians certified in medical specialties must also disclose the name of their certifying board or association using the same methods.

Written disclosures must be presented in at least 24-point type in a specified format listing name/license, highest academic degree, and board certification.

Exempts certain practitioners including those in licensed laboratories, health care facilities, podiatrists, dentists, and other specified licensed professions from disclosure requirements.

Requires practitioners with Internet websites they control to prominently display required education and certification information on those sites.
