Creates the Altruistic Living Donor Registry as a statewide registry for persons who wish to identify themselves as altruistic living kidney donors during their lifetime, to be established and maintained by a not-for-profit entity designated as the Altruistic Living Donor Registrar.

Requires the Department of Motor Vehicles to include enrollment options for the Donate Life California Organ and Tissue Donor Registry on driver's license and identification card applications beginning July 1, 2011, with both written and verbal inquiries to applicants.

Authorizes the registry to include persons who identify themselves as donors of organs and tissues other than kidneys upon a finding by the Federal Centers for Medicare and Medicaid Services and United Network for Organ Sharing that such donations are safe and without significant risk of complications.

Establishes a voluntary $2 contribution option on driver's license applications, with collected funds deposited into the Donate Life California Trust Subaccount to support organ donation programs after deducting the department's administrative costs.

Requires the Department of Motor Vehicles to transmit weekly to Donate Life California the enrollment information of applicants and to provide annual reports to the Legislature detailing contributions collected and nonidentifiable applicant demographic data.
