Establishes the Prepaid Mobile Telephony Services Surcharge and Collection Act to impose a unified surcharge on prepaid mobile telephony service purchases based on a percentage of the sales price.

Requires sellers of prepaid mobile telephony services to collect and remit the surcharge to the State Board of Equalization, which consists of the emergency telephone users surcharge and Public Utilities Commission surcharges.

Creates the Prepaid Mobile Telephony Services Surcharge Fund in the State Treasury with separate accounts for 911 emergency services and PUC programs to receive collected surcharge revenues.

Defines "prepaid mobile telephony services" to mean purchased mobile device access sold in advance in predetermined units or dollars with usage declining upon use, including telecommunications and information services.

Sunsets the prepaid surcharge collection mechanism on January 1, 2017, unless extended, with legislative intent to develop alternative collection methods for local utility user taxes and local communications-related charges by that date.
