Expands mammography coverage requirements for health care service plans and disability insurance policies to include referrals from participating physician assistants, in addition to existing nurse practitioners, certified nurse-midwives, and physicians.

Applies to health care service plan contracts (except specialized plans) and individual or group disability insurance policies issued, amended, delivered, or renewed on or after January 1, 2000.

Preserves existing provisions allowing copayment and deductible applications, and restricts coverage to participating providers unless referred by a participating physician, nurse practitioner, or certified nurse-midwife.

Specifies that coverage does not extend to specialized health insurance, Medicare supplement insurance, short-term limited duration health insurance, CHAMPUS supplement insurance, TRI-CARE supplement insurance, hospital indemnity, accident-only, or specified disease insurance.

Declares no state reimbursement is required for local agencies or school districts under Article XIII B of the California Constitution.
