Extends the skilled nursing facility (SNF) quality assurance fee assessment and Medi-Cal reimbursement rate methodology from July 31, 2013 to July 31, 2015, with repeal on January 1, 2016.

Modifies the Skilled Nursing Facility Quality and Accountability Special Fund by requiring certain set-asides to remain in the General Fund and increasing the amount of set-asides transferred to the fund beginning with the 2013-14 rate year.

Establishes supplemental quality and accountability payments to SNFs beginning with the 2013-14 rate year based on performance measures including immunization rates, pressure ulcer incidence, restraint use, staffing compliance, and resident satisfaction.

Requires nursing facilities to either provide discharge planning and referrals to local contact agencies (LCAs) for residents interested in home- and community-based services under federal Minimum Data Set assessment requirements.

Increases the annual Medi-Cal reimbursement rate for SNFs by 3 percent for the 2013-14 and 2014-15 rate years, plus costs of complying with new mandates, with a portion set aside for quality and accountability payments.
