Requires claims administrators to provide claim forms and notices of benefit eligibility to employees within 3 working days if they have knowledge the employer failed to do so, or within 30 days of knowledge of the claim if unable to determine employer compliance.

Mandates that notices to employees include explanations of options available to object to utilization review decisions that modify, delay, or deny medical treatment.

Authorizes the Workers' Compensation Appeals Board to award attorney's fees to applicants who prevail in enforcing a final permanent disability award when a medical dispute arises during the utilization review process.
