Requires parents or guardians claiming immunization exemptions for their children to document in a letter or affidavit which required immunizations have been given and which have been withheld based on their beliefs.

Mandates that beginning January 1, 2014, exemption requests must be accompanied by a state-prescribed form with a signed attestation from a health care practitioner confirming the practitioner provided information about immunization benefits, risks, and disease health risks to the parent or guardian.

Requires parents or guardians to sign a written statement confirming they received the health care practitioner's information about immunizations and communicable diseases, with both attestations signed no more than six months before the immunization requirement takes effect.

Defines "health care practitioner" to include physicians, nurse practitioners, physician assistants, osteopathic physicians, naturopathic doctors under physician supervision, and credentialed school nurses.

Allows photocopies of the signed form or letters from health care practitioners containing all required information and attestations to be submitted in lieu of the original form.
