Authorizes the Department of Health Care Services to establish demonstration projects integrating Medicare and Medi-Cal benefits for "dual eligible" beneficiaries (persons 21+ enrolled in both programs) in up to eight counties beginning no sooner than March 1, 2013.

Requires managed care health plans participating in the demonstration to identify beneficiaries age 55+ at risk of nursing home placement and notify them of potential eligibility for the Program of All-Inclusive Care for the Elderly (PACE).

Permits persons eligible for PACE to disenroll from managed care health plans and enroll in PACE at any time to receive both Medicare and Medi-Cal benefits, without restriction by enrollment lock-in periods.

Establishes that demonstration sites must meet quality standards, maintain adequate provider networks, reimburse providers at rates sufficient to ensure access, and comply with memoranda of understanding negotiated with the Centers for Medicare and Medicaid Services.

Requires the Department of Health Care Services to report annually to the Legislature on enrollment status, quality measures, and state costs of the demonstration projects.
