Authorizes the California Health Facilities Financing Authority to award grants up to $1,500,000 total to one or more projects demonstrating new or enhanced cost-effective methods of delivering quality health care services to vulnerable populations in community settings.

Grants may be allocated in increments over multiple years, with subsequent allocations contingent on evidence that projects are progressing toward identifying a high-quality, cost-effective, replicable delivery model.

Creates the California Health Access Model Program Account within the California Health Facilities Financing Authority Fund and transfers up to $1,500,000 to the account, with any remaining balance reverting to the fund on January 1, 2015.

Requires the authority to report to the Legislature and Governor by January 1, 2014, on demonstration grant program outcomes, including total and individual grant amounts, funding sources, project descriptions, and demonstrated cost-effectiveness and health outcomes.

Grant recipients must comply with applicable laws regarding scope of practice, licensure, staffing, and building codes, and cannot claim authority funding toward community benefit and charity care obligations.
