Allows the State Department of Public Health to designate another eligible applicant as the lead agency to respond to federal, state, local, or nonprofit funding opportunities and solicitations.

Permits the department to enter into memoranda of understanding or agreements with local health departments, institutions of higher education, nonprofit organizations, or entities formed by multiple organizations to serve as lead agencies.

Requires lead agency designees to agree in advance to specified terms and conditions including full compliance responsibility, intellectual property permissions, indemnification of the state, and cancellation provisions.

Establishes eligible public health project goals including creation of healthier environments, infrastructure for active living and nutritious food access, age-targeted health programs, worksite wellness assessment, and strategies to address health disparities.
