Creates a "retired license" for optometrists with a current active license, issued upon application and payment of a fee not to exceed $25, which prohibits the holder from practicing optometry but allows use of titles "retired optometrist" or "optometrist, retired."

Establishes a "retired license with volunteer service designation" for optometrists to provide voluntary, unpaid services at health fairs and vision screenings, with initial issuance and biennial renewal fees not to exceed $50, subject to continuing education requirements.

Allows holders of retired licenses issued for less than three years to reactivate to active status under Section 3147 requirements, and holders of retired licenses issued for more than three years to reactivate under Section 3147.6 requirements.

Requires optometrists to obtain a statement of licensure from the board for all practice locations except their principal place of practice, while exempting those engaging in "temporary practice" (no more than 5 calendar days per 30-day period, 36 days per year) from address notification requirements.

Specifies that the State Board of Optometry's enforcement and disciplinary authority applies to holders of retired licenses, licenses with retired volunteer designation, and inactive licenses.
