Adds Section 56.08 to the Civil Code requiring health care providers, plans, and contractors to offer one year of free credit monitoring services when breaching security of a patient's social security number, driver's license number, California identification card number, or financial information (credit/debit card numbers)

Credit monitoring services must be offered in the breach notification required under Section 1798.82 or applicable federal law, and must be provided if the patient accepts the offer

Establishes this requirement as a state-mandated local program and specifies that no reimbursement is required because costs result from creating new criminal penalties under the Confidentiality of Medical Information Act
