Establishes an Independent Medical Review System allowing patients to seek external medical reviews when health care services are denied, modified, or delayed by health plans or insurers based on medical necessity determinations.

Effective July 1, 2015, requires independent medical reviewers to be clinical experts with recent or current experience treating similar conditions, replacing previous "knowledgeable clinician" standard.

Modifies application forms from one page to one or two pages and adds optional demographic data collection (ethnicity, race, primary language) for statistical purposes only.

Requires director decisions to be published in searchable, no-cost online databases by July 1, 2015, including detailed case information, reviewer credentials, demographic data, and aggregate statistics by plan and ethnicity/race/language.

Applies to both health care service plans (Health and Safety Code) and disability insurers (Insurance Code) with identical requirements and effective dates.
