Requires transmission of health care application information to county human services departments to simultaneously initiate CalWORKs and CalFresh applications when applicants consent on the single state health application form.

Authorizes the Secretary of California Health and Human Services to phase in implementation if necessary to ensure timely and accurate decisions on applications for aid, without reducing program eligibility.

Mandates the California Health and Human Services Agency to convene a workgroup by July 1, 2013 to study integrating additional human services and work support programs with the single state application, including feasibility, costs, and benefits.

Allows the Secretary to declare the cross-application process inoperative if implementation would prevent timely rollout of the standardized health subsidy application, with automatic repeal two years after the inoperative date unless extended.

Requires reimbursement to local agencies for state-mandated costs through existing statutory procedures if the Commission on State Mandates determines such costs exist.
