Mandates Medi-Cal managed care plans cover these drugs if the prescribing provider demonstrates medical necessity and consistency with FDA labeling and use rules supported by official compendia.

Allows managed care plans to exclude these drugs by carving them out and covering them on a fee-for-service basis instead, with corresponding reductions to the plan's contracted rate.

Establishes legislative intent that prescriber professional judgment prevails for covered therapeutic drug classes that lack formulary coverage or have prior authorization requirements.

Applies only to the extent permitted by federal Medicaid law.
