Requires HIV/AIDS public health record disclosures to include only necessary information and be made with agreements to maintain confidentiality, with written authorization required except in specific circumstances.

Authorizes disclosures to CDC for coinfection reporting with tuberculosis, syphilis, gonorrhea, chlamydia, hepatitis B, hepatitis C, and meningococcal infection through designated public health agency staff.

Permits local public health agency communicable disease, sexually transmitted disease control, and tuberculosis staff to further disclose information to state or local counterparts for facilitating appropriate medical care and treatment of persons with HIV coinfections.

Establishes civil penalties of up to $5,000 for negligent disclosure and $5,000-$25,000 for willful or malicious disclosure, plus potential misdemeanor charges for disclosures causing economic, bodily, or psychological harm.

Prohibits use of disclosed confidential public health records to determine employability or insurability.
