Modifies birth certificates effective January 1, 2016, to replace "mother" and "father" designations with two "Name of Parent" lines, each with checkboxes for "Mother," "Father," or "Parent" to describe parental relationship to the child.

Requires all local registrars, deputy registrars, and subregistrars to update forms to incorporate the modification and discard all previous forms in use before January 1, 2016.

Allows parents of children born before January 1, 2016, to amend existing birth certificate titles "Mother/Parent" and "Father/Parent" to one of the new parent relationship designations through the standard amendment process.

Requires the birth mother's name, date of birth, and place of birth to be linked to her medical and social information on the certificate, with this linkage kept confidential and accessible only to authorized parties.

Repeals prior birth certificate requirements and makes conforming changes to vital records statutes, with all new provisions becoming operative January 1, 2016.
