Requires state agencies using electronic forms on Internet websites to display a disclosure notice in 12-point boldface type directly above the submit button, stating that information collected may be shared with other state agencies or private parties in accordance with California law.

Prohibits state agencies from utilizing or sharing any information from electronic forms until the person submitting the form specifically clicks the submit button.

Defines "state agency" to include every state office, officer, department, division, bureau, board, and commission, including the California State University.

Specifies that information in partially completed forms that have not been formally submitted shall not be utilized or shared.
