Establishes the Office of Community Care Coordination within the State Department of Developmental Services, to be located on the grounds of the Sonoma Developmental Center.

Requires the office to develop a plan by January 1, 2016 that includes operating at least 2 acute crisis clinics with 15 or fewer beds each in northern and southern California, and establishing transitional and residential facilities with wrap-around services.

Mandates the office collaborate with regional centers to expand mobile crisis response teams, crisis hotlines, day programs, and create short-term homes.

Requires the office to define "placement of last resort" and "acute crisis clinic" and identify modifications needed for Sonoma Developmental Center to operate as both a placement of last resort and acute crisis clinic.

Plan must be consistent with the California Health and Human Services Agency's master plan for the future of developmental centers and address community capacity and staff transitions.
