Creates the Office of Farm to Fork within the Department of Food and Agriculture to coordinate efforts in increasing agricultural product access to underserved communities and schools statewide.

Requires the office to identify communities lacking healthy food access, coordinate with local and state agencies, promote retail sales of healthy food, and foster partnerships between farmers, businesses, nonprofit organizations, and food banks.

Establishes the Farm to Fork Account in the Department of Food and Agriculture Fund to receive and continuously appropriate money from federal, state, industry, philanthropic, and private sources without regard to fiscal years.

Requires annual reporting to the Legislature on the Farm to Fork Account's income and expenditures for any fiscal year in which funds are received or expended, until the reporting requirement expires on January 1, 2020.
