Transfers the Advance Health Care Directive Registry from the Secretary of State to the State Department of Public Health, effective January 1, 2016, and renames it the California Health Care Decisions Online Registry.

Requires the registry to maintain secure online access allowing individuals to view, download, print, and retrieve electronic copies of advance health care directives, physician orders for life-sustaining treatment forms, durable powers of attorney, and other health care decision documents.

Establishes procedures for registering, replacing, and removing advance health care directives, issuing registration cards with access numbers and passwords, and verifying identities of health care providers and authorized persons requesting information.

Requires the registry to be fully self-supporting through registrant fees by January 1, 2020, and mandates submission of a financial sustainability plan to the Legislature by January 1, 2018.

Directs the Department of Public Health to contract with the University of California, Davis to develop and maintain the registry and to work with the Attorney General's Office to provide information about end-of-life care and advance directives.
