Requires the State Department of Health Care Services to request a federal waiver to allow Medi-Cal to claim federal financial participation for health home services provided to state or county inmates during their last 30 days in custody.

Health home services for prerelease inmates limited to care coordination and related services as specified in existing law, excluding direct health care services.

Establishes legislative intent to ensure individuals in the criminal justice system who are otherwise Medi-Cal eligible (including those on probation, parole, diversion programs, and postrelease supervision) have access to Medi-Cal benefits.
