Requires the Division of Oil, Gas, and Geothermal Resources to develop a standardized form for notifying surface property tenants and owners about well stimulation treatment permits, prepared in English and Spanish in plain language.

Mandates that the independent entity or person conducting notifications provide the Division with a copy of all notifications delivered to surface property tenants, owners, or authorized agents.

Makes failure to provide notification copies to the Division a criminal offense.

Specifies that no state reimbursement is required because costs stem from creating a new crime under existing Government Code provisions.
