Increases the maximum length of the independent medical review notification form from 1 page to 2 pages, with the form prescribed by the administrative director to include specific information about employee rights and the utilization review decision.

Expands the types of liens the Workers' Compensation Appeals Board may allow to include medical-legal expenses to which employees are entitled under Section 4621 for proving or disproving disputed claims.

Clarifies that lien filing fees and lien activation fees must be paid by the employer of the injured worker and specifies conditions under which lien claimants are entitled to reimbursement with interest.

Permits assignment of lien payment to an assignee for assignments completed prior to January 1, 2013, or required by a contract that became enforceable and irrevocable prior to January 1, 2013, and declares this to be existing law.

Removes provisions relating to vocational rehabilitation services from the statute allowing injured workers to institute proceedings within 5 years of injury for collection of compensation based on new and further disability caused by the original injury.
