Amends Labor Code Section 6409.1 to require employers to report serious injuries, illnesses, or deaths to the Division of Occupational Safety and Health immediately by telephone or email (previously allowed telephone or telegraph only).

Employers must file complete written reports of occupational injuries or illnesses within five days of obtaining knowledge, with the Department of Industrial Relations or their insurer.

Employers who violate the immediate reporting requirement for serious injuries, illnesses, or deaths may be assessed a civil penalty of not less than $5,000.

Employers must file amended reports within five days if an employee dies as a result of a previously reported injury or illness.

Insured employers' insurers must file copies of injury/illness reports with the division immediately upon receipt.
