Legislature supports and encourages development and expansion of patient centered medical homes as a health care delivery model based on care coordination principles.

Defines "patient centered medical home" as a model where patients establish ongoing relationships with primary care physicians or licensed health care providers in physician-led practice teams to provide comprehensive, accessible, continuous evidence-based care and coordinate health care needs across the system.

Establishes seven principles for patient centered medical homes including collaborative team approaches, continuous care access, care coordination across mental health and substance use disorder services, health information technology utilization, and payment structures that reward appropriate care delivery and discourage over- or under-provision of services.

Requires incorporation of seven quality and safety components including patient advocacy, evidence-based medicine, accountability for quality improvement, active patient participation, appropriate information technology use, family participation in quality improvement, and enhanced patient access to health care.
