Applies the same identity-verification notice requirement to jurats attached to sworn affidavits.

Establishes specific required forms for certificates of acknowledgment, proof of execution, and jurats that include the boxed notice at the top.

Creates a civil penalty of up to $10,000 for notaries public who willfully state material facts they know to be false, enforceable by the Secretary of State or public prosecutors.
