Requires health care service plans and disability insurers to provide coverage for all FDA-approved contraceptive drugs, devices, and products without cost-sharing for women effective January 1, 2016.

Mandates coverage of voluntary sterilization procedures, contraceptive education and counseling, and related follow-up services including device insertion and removal.

Prohibits non-grandfathered health plans from imposing deductibles, copayments, or other cost-sharing restrictions on contraceptive coverage.

Allows religious employers to request exemptions from contraceptive coverage if it conflicts with their religious tenets, provided they give written notice to prospective enrollees.

Extends contraceptive coverage requirements to Medi-Cal managed care plans and modifies utilization controls for family planning services to comply with cost-sharing prohibitions.
