Changes sustainability grant amounts from $25,000-$125,000 to $50,000-$100,000 on a one-time basis and revises their use to focus on developing sustainable funding models through leveraging existing funding streams.

Establishes new population health grants of $50,000-$125,000 for up to 3 years to fund interventions addressing specific health risk factors including obesity prevention, asthma prevention, mental health intervention, and substance abuse prevention.

Maintains funding for planning grants ($25,000-$50,000), facilities and startup grants ($20,000-$250,000 per year for 3 years), and technical assistance grants through competitive bidding to support grantees.

Requires preference in grant funding for schools in medically underserved areas, with high percentages of low-income and uninsured students, limited English proficient students, and schools with low academic performance rankings.
