Requires state agencies and departments to include a signed statement from their head with written reports submitted to the Legislature, Members of the Legislature, or state legislative/executive bodies, certifying that factual contents are true, accurate, and complete to their knowledge

Applies to heads of all state agencies and departments, including elected state officials and constitutional officers, with specific provisions for the Franchise Tax Board executive officer and State Board of Equalization executive director

Defines "written report" as documents statutorily required to be submitted to the Legislature or state bodies, or documents/summaries/statements requested by individual Members of the Legislature

Exempts forecasts, predictions, recommendations, and opinions from the certification requirement, applying only to factual contents

Establishes civil penalty of up to $20,000 for any person who knowingly declares false material matters in the signed statement, recoverable through civil action by the Attorney General
