Requires the California Health and Human Services Agency to establish and operate a statewide California POLST Registry by January 1, 2016 to collect and disseminate Physician Orders for Life Sustaining Treatment forms from health care providers.

Health care providers must submit completed POLST forms to the registry unless a patient or their health care decisionmaker opts out of participation.

Requires the agency to adopt rules covering electronic submission and revision of forms, identity verification of authorized users, confidentiality protections, and methods for disseminating information to authorized users including health care providers.

Grants patients and their health care decisionmakers the ability to review, confirm accuracy, amend, or withdraw POLST forms from the registry, with confirmation of receipt required.

Provides immunity from criminal prosecution, civil liability, and professional discipline for authorized users acting in good faith based on registry information.
