Authorizes the Secretary of State to designate an alternate physical address for victims of domestic violence, sexual assault, or stalking who are prohibited from using a post office box as an address.

Requires applicants to apply in person at a community-based victims' assistance program and meet with a victims' assistance counselor.

Requires applicants to provide a sworn statement confirming victim status and fear for safety, along with supporting documentation such as police records, program documentation, or professional assistance records.

Certifies approved applicants as program participants for four years, during which the Secretary of State serves as their agent for service of process and mail receipt.

Establishes penalties of misdemeanor charges for applicants who falsely attest that disclosure of their address would endanger safety or provide false information on the application.
