Establishes the California Childhood Immunization Quality Improvement Fund (CCIQIF) program administered by the State Department of Health Care Services to improve childhood immunization rates among Medi-Cal beneficiaries.

Requires the department to submit a waiver or demonstration project application to federal Centers for Medicare and Medicaid Services within 270 days, with the demonstration project operating for five years.

Allocates 33.3 percent of CCIQIF expenditures for provider support and administrative costs, and 66.7 percent for reward payments of $125 per Medi-Cal enrollee who receives all recommended vaccinations by age two.

Requires at least 20 percent of reward payment expenditures to be passed through to contracted providers based on their Medi-Cal enrollment of children under two years old.

Mandates an independent evaluation of the program's effectiveness using data from the first three years, to be submitted to the Legislature within the fourth year of the demonstration project, financed with no more than five percent of annual CCIQIF expenditures.
