Requires the Department of Social Services to create and maintain the Excluded Persons Administrative Action List (EPAAL) on its website by January 1, 2017, containing all individuals subject to administrative actions (license revocation, denial, probation, or exclusion) since January 1, 1990, updated every 30 days.

Mandates licensees post the department's unredacted exclusion notice next to the facility license within 24 hours of receiving notice of an excluded person and provide written notice to residents, responsible parties, and local ombudsmen within 10 days.

Establishes failure to check the EPAAL or post the department's exclusion notice as serious violations subject to an immediate civil penalty of $150 per day per violation.

Requires placement agencies to report the presence of individuals listed on the EPAAL as a reportable incident that jeopardizes resident health or safety.

Requires the department to conduct unannounced visits within 30 days of serving an exclusion order to verify the exclusion notice has been posted at the facility.
