Allows requestors unable to establish identity electronically to submit a notarized statement of identity as an alternative method.

Maintains existing requirements for written, faxed, and in-person requests, including notarized statements sworn under penalty of perjury for authorized persons.

Specifies that law enforcement and funeral establishment employees remain exempt from providing notarized statements when requesting records in official capacity or scope of employment.
