Requires the State Department of Social Services to develop and implement a foster parent evaluation process in consultation with county welfare directors, foster parents, caregivers, and current and former foster youth.

Mandates that foster youth over 12 years of age and nonminor dependents provide feedback on care quality in licensed or certified foster homes and group homes at least once per year and upon exit.

Requires development of an evaluation tool assessing caregiver performance including case plan implementation, family integration, honoring of youth rights, support for biological family relationships, advocacy, health care coordination, educational support, life skills development, and transition assistance.

Directs the department to consider how evaluation information can improve recruitment, training, and retention of high-quality foster parents.

Requires full implementation of the evaluation process and adoption of all necessary regulations by January 1, 2017.
