Authorizes applicants and recipients of In-Home Supportive Services (IHSS) to designate an authorized representative in writing on a state-developed form to accompany, assist, and represent them in matters including applications, service direction, and eligibility redetermination.

Requires the authorized representative form to specify an effective time period, responsibilities to be performed, and be revocable or changeable at any time by the applicant or recipient.

Prohibits individuals convicted of fraud against government health care or supportive services programs from serving as authorized representatives, with exceptions for court-appointed guardians, conservators, and parents of minors.

Allows authorized representatives to sign timesheets and provider-related documents on behalf of recipients if specified on the form, except that provider-authorized representatives cannot sign their own timesheets unless they are a legal guardian or conservator.

Requires counties to retain the original authorized representative form in the recipient's IHSS case file and provide copies to the applicant/recipient and the authorized representative; directs the State Department of Social Services to develop a standardized form and adopt emergency regulations by July 1, 2016.
