Department of Health Care Services shall apply for an additional 5,000 waiver slots by February 1, 2016, to be added in fiscal year 2016-17 for the home- and community-based Nursing Facility/Acute Hospital Waiver.

One-time community transition services (up to $5,000) and habilitation services shall be added to the waiver, including security deposits, furnishings, moving expenses, and utility setup fees.

Department shall expedite waiver application processing for patients in acute care hospitals pending nursing facility placement, requiring eligibility determinations and service notifications within three business days.

Individuals enrolling in the waiver at age 21 shall receive at least the same level of services they received through the EPSDT program, with no reduction in in-home nursing services.

Implementation requires federal financial participation and demonstration of fiscal neutrality within the department budget and under federal waiver terms.
