Requires the State Department of Public Health to provide parents/guardians with signed acknowledgment forms confirming receipt of information about storage, retention, and use of newborn blood samples for medical research.

Authorizes parents/guardians of minors and individuals age 18 and older to request that their newborn blood samples be destroyed or not used for research purposes, with the department required to comply and send email confirmation if an email address is provided.

Mandates the department prepare single-page informational materials explaining the newborn screening test, blood sample retention and research use, and a separate standard informational acceptance form to be signed by parents/guardians.

Requires birth attendants and perinatal health facilities to distribute informational materials and acceptance forms to pregnant women, and requires local registrars of births to provide these documents to those registering births outside health facilities and notify local health officers.

Establishes state reimbursement procedures for any costs mandated to local agencies under the California Constitution.
