Prohibits juvenile courts from terminating jurisdiction over wards age 17 or older until conducting a hearing and confirming the probation department has provided specified information, documents, and services to the ward.

Requires probation departments to provide or make reasonable efforts to provide: written case information, medication details, social security card, birth certificate, health/education summaries, driver's license or ID, a probation letter, health insurance assistance, housing referrals, employment referrals, educational assistance, contact information for important individuals, juvenile record sealing information, and immigration status updates if applicable.

Allows juvenile courts discretionary authority to apply these provisions to wards under 17 years of age if requested by the ward and found to be in the ward's best interest.

Permits wards to waive their right to receive the required information, documents, and services after conferring with counsel, and prohibits delaying hearings or holding wards in confinement solely to comply with these requirements.

Directs the Judicial Council to develop standards and forms necessary to implement these provisions and establishes state reimbursement procedures for any mandated local costs.
