Adds comprehensive medication management (CMM) services as a covered benefit under the Medi-Cal program, defined as a process to ensure medications are appropriate, effective, and safe for each beneficiary.

Requires CMM services be offered to beneficiaries taking three or more prescription drugs for chronic conditions, those recently discharged from hospitals/care facilities, those referred by prescribers, or meeting other department-established criteria.

CMM services must include health status assessment, documentation of clinical goals, medication evaluation, identification of therapy problems, development of a written treatment plan, patient education, and follow-up monitoring, typically involving three to four visits per year.

Services must be provided by primary care physicians or pharmacists working collaboratively with other providers and directly communicating with beneficiaries.

Requires the Department of Health Care Services to evaluate CMM effectiveness on quality of care, patient outcomes, total program costs, and any savings attributable to the coverage.
